It looks pretty much like this. The back is a giant scroll for collecting dead bodies and the dots on the head are indents for the Pokey Head Spider to perch.
Abilities: The puppet can use any elemental technique(including advanced element) known by the wielder, even simultaneously, but the latter is costly as no chakra discount is provided so you will indeed be paying double to use the technique. It should also be noted that if you try to use any non-elemental techniques other than Kugujutsu with their free hand the puppet will become unresponsive for 3 posts.
